Good Afternoon,
We want to make sure our students are prepared for the upcoming school year. If your child has daily medications, important medical diagnoses or conditions, or you'd like them to have medication on hand for an "as needed" basis, please ensure you have completed the following as soon as possible:
FORMS:
Print & fill out any necessary documents, which can be found HERE.
Bring in all forms and medications to the front office as soon as possible, ideally during Open House on July 30th. However, we will be ready to accept all medications and forms July 29-31, and August 3-7, to ensure a smooth start to the school year.
If anything changes during the school year, you may submit updated forms at any time. To make sure all teachers and staff are on the same page regarding your student's medical needs, please do your best to get forms as soon as possible.
OVER THE COUNTER:
Any OTC medications must be in an unopened package. This includes ibuprofen, acetaminophen, cough drops, etc.
Ensure you have purchased medication that is instructed to be given to your child based on their age. For example, if your student is 12 years old, they cannot take children's medication if the package dosing instructions are only for children up to age 10.
SELF CARRY:
If you intend to have your child carry any medications with them, please make sure you fill out the self carry form, which can be found HERE.
